Purpose:

To implement assist with oversee the delivery of comprehensive services to Adults and their families who have been impacted by serious mental illness/co-occurring disorders, which places them at risk for hospitalization.

 

Primary Responsibilities:  

• Conducts comprehensive bio-psychosocial assessments accordance with agency assessment protocol.

• Engage in at least 15 to 20 direct hours with individuals assigned via Community Crisis Stabilization supports. 

• Partners with staff and clients to develop and oversee the implementation of person-centered recovery-based treatment plans.

• Review clinical documentation such as progress notes, discharge summaries, etc.   

• Assists with crisis intervention as applicable. 

• Provide individual and family counseling under the direct supervision of an LMHP.

• Actively coordination case management, discharge, and aftercare planning with referral agencies

• Conduct weekly supervision with counselors as assigned. 

• Manage deadlines and submission of individualized service plans, authorizations, re-authorizations etc.  

• Works collaboratively with a multidisciplinary treatment team and community-based partners.  

• Engage in weekly supervision with LMHP.
