Clinical Case Manager
The Full Story
Purpose:
To implement assist with oversee the delivery of comprehensive services to Adults and their families who have been impacted by serious mental illness/co-occurring disorders, which places them at risk for hospitalization.
Primary Responsibilities:

Conducts comprehensive bio-psychosocial assessments accordance with agency assessment protocol.

Engage in at least 15 to 20 direct hours with individuals assigned via Community Crisis Stabilization supports.

Partners with staff and clients to develop and oversee the implementation of person-centered recovery-based treatment plans.

Review clinical documentation such as progress notes, discharge summaries, etc.

Assists with crisis intervention as applicable.

Provide individual and family counseling under the direct supervision of an LMHP.

Actively coordination case management, discharge, and aftercare planning with referral agencies

Conduct weekly supervision with counselors as assigned.

Manage deadlines and submission of individualized service plans, authorizations, re-authorizations etc.

Works collaboratively with a multidisciplinary treatment team and community-based partners.

Engage in weekly supervision with LMHP.
