Qualified Mental Health Professional- Adult 

The Full Story

Purpose: To provide direct mental health care to non-hospitalized adults 18 and up in a residential setting experiencing an acute crisis of a psychiatric nature that may jeopardize their current community living situation.  

 

Primary Duties:

  • Work with adults with mental health and co-occurring challenges to assist with addressing immediate mental health, medical and or psychosocial skills needs to reduce crisis.

  • Work with population served to engage in care coordination and discharge planning for the purposes of connecting to resources for ongoing recovery and management.  

  • Actively participate in the client intake process as well as follow the crisis program curriculum. 

  • Engage in the ongoing assessment of the client’s treatment needs for person centered treatment planning.

  • Assist in the development and ongoing monitoring and implementation of Individual Service Plan. 

  • Complete psycho-education interventions hourly to assist the client with gaining skills needed to reduce and managing presenting behaviors.

  • Work with clients to foster the development of new improved skills such as but not limited to daily living skills, coping skills, problem solving skills, communication skills, health relationship skills.

  • Maintain up-to-date client records and prepare required/necessary reports in a timely and accurate manner such as individualized service plans, daily progress notes and discharge summaries.

  • Establish a good working relationship with other public and private agencies/services and interface collaboratively with them
