The Full Story
Purpose: To provide direct mental health care to non-hospitalized adults 18 and up in a residential setting experiencing an acute crisis of a psychiatric nature that may jeopardize their current community living situation.
Primary Duties:

Work with adults with mental health and co-occurring challenges to assist with addressing immediate mental health, medical and or psychosocial skills needs to reduce crisis.

Work with population served to engage in care coordination and discharge planning for the purposes of connecting to resources for ongoing recovery and management.

Actively participate in the client intake process as well as follow the crisis program curriculum.

Engage in the ongoing assessment of the client’s treatment needs for person centered treatment planning.

Assist in the development and ongoing monitoring and implementation of Individual Service Plan.

Complete psycho-education interventions hourly to assist the client with gaining skills needed to reduce and managing presenting behaviors.

Work with clients to foster the development of new improved skills such as but not limited to daily living skills, coping skills, problem solving skills, communication skills, health relationship skills.

Maintain up-to-date client records and prepare required/necessary reports in a timely and accurate manner such as individualized service plans, daily progress notes and discharge summaries.

Establish a good working relationship with other public and private agencies/services and interface collaboratively with them
