Intensive Outpatient Services (ASAM Level 2.1)

Mobile Crisis Response provides rapid response, assessment, and early intervention to individuals experiencing a behavioral health crisis.  This service is provided 24 hours a day, seven days a week.  The purpose of this service includes prevention of acute exacerbation of symptoms, prevention of harm to the individual or others, provision of quality intervention in the least restrictive setting, and development of an immediate plan to maintain safety in order to prevent the need for a higher level of care. 
 

Mobile Crisis Response is designed to support individuals in the following manner:

  • Provide rapid response to individuals experiencing a crisis situation or escalating emotional/behavioral symptoms which have impacted the individual’s ability to function in their family, living situation, community, school, or work/ environment;

  • Meet individuals in crisis in an environment where they are comfortable to engage to facilitate quick relief and resolution of the crisis when possible;

  • Provide appropriate care/support/supervision in order to maintain safety for the individual and others, while avoiding unnecessary law enforcement involvement, emergency room utilization, and/or avoidable hospitalization;

  • Refer and link to all medically necessary behavioral health services and supports, including access to appropriate services along the behavioral health continuum of care (including pre-admission screening in appropriate cases conducted by a DBHDS certified pre-screener);

  • Coordinate with behavioral health providers providing services to the individual throughout the delivery of the service;

Deployed in real-time to the location of the identified crisis.
 

 

Community Stabilization services are available 24 hours a day, seven days a week, to provide for short-term assessment, crisis intervention, and care coordination to individuals experiencing a behavioral health crisis.
 

Services may include brief therapeutic and skill-building interventions, engagement of natural supports, interventions to integrate natural supports in the de-escalation and stabilization of the crisis, and coordination of follow-up services.

Services involve advocacy and networking to provide linkages and referrals to appropriate community-based services and assisting the individual and their natural support system in accessing other benefits or assistance programs for which they may be eligible.

 

The goal of Community Stabilization services is to stabilize the individual within their community and support the individual and natural support system during the following:

1) between an initial Mobile Crisis Response and entry into an established follow-up service at the appropriate level of care if the appropriate level of care is identified but not immediately available for access

2) as a transitional step-down from a higher level of care if the next level of care is identified but not immediately available or

3) as a diversion from a higher level of care. Critical Features of Community Stabilization include:

  • Peer recovery support services

  • Health literacy counseling 

  • Skills restoration
     

Mental health skill-building services (MHSS) shall be defined as goal-directed training and supports to enable restoration of an individual to the highest level of baseline functioning and achieve and maintain community stability and independence in the most appropriate, least restrictive environment. MHSS services shall provide face-to-face activities, instruction, interventions, and goal-directed training that are designed to restore functioning and that are defined in the ISP in order to be reimbursed by Medicaid. MHSS shall include goal-directed training in the following areas: (i) functional skills and appropriate behavior related to the individual’s health and safety; instrumental activities of daily living, and use of community resources; (ii) assistance with medication management; and (iii) monitoring health, nutrition, and physical condition with goals towards self-monitoring and self-regulation of all of these activities.

 

MHSS services include the following components:  Providing opportunities to enhance recovery plans that include but are not limited to: o Daily living activities and training on personal care/hygiene to restore and regain functional skills and appropriate behavior related to health and safety; and, o Skills training and reinforcement on the use of available community resources, such as public transportation to improve daily living and community integration skills and independent use of community resources, etc.
 

Intensive Outpatient Services (ASAM Level 2.1)

Intensive outpatient services (ASAM Level 2.1) as defined in 12VAC30-130-5090 and 12VAC35- 105-1730 to 1770 shall be provided by an interdisciplinary team of CATPs, which may include generalist physicians or physicians with experience in addiction medicine. Intensive outpatient services are structured program of skilled treatment services for adults, children and adolescents delivering a minimum of 3 service hours per service day to achieve an average of 9 to 19 hours of services per week for adults and an average of 6 to 19 hours of services per week for children and adolescents. Services can also be provided on the weekend and those hours are part of the weekly hours as long as they do not exceed the standard weekly hours. This service is provided to members who do not require the intensive level of care of inpatient, residential, or partial hospitalization services, but requires more intensive services than outpatient services.
